If you have followed all the steps to create your own Evan's Box (made contact with a person at your chosen location, found a box with a lid, and filled the box with micro baby items), you are now ready to request your Evan's Box decal!

PLEASE NOTE: In order to ensure the quality of Evan's Boxes and accuracy on our locations list, we are unable to accept requests that do not have a location contact name with an email address or phone number provided. Please do not put the location name and phone number in the "contact" lines. Incomplete requests received, or requests that do not follow these guidelines, will not be accepted.
